Welcome to the University of Florida Center for Couples and Families. We offer free counseling to help improve the relationships. The clinic is part of the UF counselor education professional training program at Norman Hall.

About Us

Staffed by UF faculty, doctoral students and advanced counseling students, our clinic specializes in couples and family therapy. The clinic’s services are free of cost and open to all.

How We Work

We take new clients on a first-come, first-served basis. If there are no openings for prospective patients, we will either put you on a waiting list or recommend alternate resources.

Our intake process involves the following steps. First, we will talk via phone to ensure our services match your needs. Once we schedule the appointment, we ask that all key people involved attend the sessions. This approach allows us to learn more about what is happening, what has been tried and what is needed. Our counseling sessions are scheduled for 60 minutes weekly during the semester, and you meet with the same counselor each week. Transfers are possible from semester to semester if deemed necessary.

At the UF Couple and Family Clinic, we have observed that clients experiencing personal, couple or family-related difficulties are more effectively helped when family members are involved in the counseling process. Because of this, we invite families and significant others to be involved.

Information and Expectations

We do not charge fees for the services we offer our clients. However, we ask for your consent to video your counseling sessions. We tape all sessions so clinic staff can provide feedback to the counselors. This approach helps us provide you with the highest quality service.

Videotaping and live supervision enable trainers to watch and consult with teaching staff, helping to improve the counseling process. Our policy is to erase all session videos after one year from the completion of the counseling relationship.

Our team consultation approach facilitates additional feedback for your counselor(s) as they engage with you during sessions. During your session, your provider takes a short break to consult with the team.

We will also ask you to complete a few forms when you begin with us, as well as at the end of the semester. This information is used to better understand what you’re bringing to us and discover ways we might be more effective in helping others.

Clients are asked to sign a form confirming their agreement to participate in the clinic.

We ask that you keep appointments. If you are unable to keep an appointment, please call us. If you need to cancel, we ask you to give us at least a 24-hour notice.

Missing appointments without calling and more than two cancellations may result in termination of services, as we generally maintain a waitlist.

You can expect that any information shared will be considered confidential. We place a high value on what you share with us.

New Clients

Forms – here you will find the forms that we will need for your first session.  Please download and complete the forms, bringing the completed forms along with you for your first session with your counselor.

Forms

Prior to your first visit, there are several forms you can complete and bring with you. These forms are required to be completed before you can be seen as a client in the clinic. The forms both provide quality information to your counselor and cover some of the more legal aspects of our clinic to meet HIPPA requirements. These are put in place to provide you the highest quality service and to protect you as a client.IMG_7397
The forms are offered in two versions for your convenience. The first is an Adobe Acrobat file (.pdf) which will allow you to print out the form and fill it out with a pen. And the second is a Microsoft Word document (.doc) that will allow you to fill in the information in a document format. Please choose the format that is the most convenient for you.

Questionnaire

Prior to scheduling an appointment, the clinic needs to gather information from you to match you and your partner or family to best possible services. To begin this process, we ask for the information contained on this form. After completing the form, you are welcome to email it to the Clinic Director, Dr. Amber Moss at ambermoss@ufl.edu.

Psychosocial History

This form is to give your counselor an understanding of who you are, some biographical information, some social and familial history. Overall, giving them a snapshot on paper of the three major realms (biological, psychological and sociological) to best understand how to help you.

Informed Consent

This form is required by the University, the state of Florida and all professional organizations. It ensures that you are aware of the treatment process, your rights, the limitations of therapy and consent to engage in the process. Your counselor will review the form with you during your first session and be happy to answer any questions you have over the process.
